School-Based Outreach Worker (SBOW) will conduct outreach activities to increase community awareness of the WCHC SBHC Program and recruit eligible participants into the Program, focusing on the target population who lives in Mount Vernon, NY, including school personnel, parents, and students. The position involves providing consistent, self-serving, and quality customer service to internal staff members and external partners to support our school-based health services.
The Outreach Worker will actively manage assigned outreach events, enhance the customer experience, and build and maintain relationships in the region they support. This position includes but is not limited to attending outreach events, health fairs, school activities, parent-teacher night, calendar management, patient follow-up, assistance in the enrollment process, language translation, insurance verification, and coordination with the school-based health staff.

Essential Duties:

  • Develops effective working relationships with students, school personnel, and/or families.
  • Communicates with students, families, health care team, and school personnel regarding student progress.
  • Provides presentations based on school environment/climate, promotion of services, data trends, and other concentrations identified by students, families, or staff, as performance improvement initiatives.
  • Conduct activities that will facilitate comprehensive, quality services for free-reduced families.
  • Participate in meetings, workshops/training, outreach, and community activities.
  • Participate in quality assurance, evaluation, and data collection required by the grant.
  • Assist with the creation of advertisements and data collection.
  • Maintains confidentiality and complies with professional ethics according to departmental and professional standards.
  • Other duties as required.
  • Outreach:
  • Works closely with the school’s liaison consultant
  • Provides follow-up and support to the family through the enrollment process.
  • Provide tracking of participants that School-Based staff is unable to reach.
  • For uninsured families, contact parents for an appointment to discuss insurance for children.
  • Attends school functions, health fairs, parent groups, and community events to promote insurance for children.
  • Liaise with the leaders of after-school programs, churches, and other related local community sites.
  • Track and document outreach encounters.
  • Enrollment:
  • Serves as a member of the Enrollment Core Team.
  • Reviews each new enrollment form for completion of information.
  • Contacts parent or school records to obtain accurate information.
  • Ensures that insurance information is complete.
  • Responsible for the timely processing of enrollment forms.
  • Distribute enrollment forms and flyers to parents and families.
  • Provides follow-up and support to families through the enrollment process

Qualifications:

  • Associate degree required.
  • Bilingual skills Preferred.
  • Proficient in Microsoft Office, PowerPoint, Outlook, and word processing
  • Proven ability to multi-task in a fast-paced work environment
  • Detail-oriented and strong organizational skills
  • Problem-solving and critical thinking skills a plus
  • Ability to facilitate referrals to support services.
  • Ability to work in a team and independently.
  • Valid driver’s license
  • Must have your own reliable transportation.
  • Must be available for evening and weekend hours as needed.
  • PHYSICAL REQUIREMENTS/WORK ENVIRONMENT:
  • Combination of sustained periods of sitting, walking, and stair climbing
  • Minimal lifting and carrying activities.
  • Ability and willingness to travel to required training and annual meetings.
  • COMMUNICATION SKILLS:
  • This position must interface with school staff, patients, family members, community agencies, and other providers within the organization.
  • Strong communication and customer service skills.
